VATSIM/IVAO flight planning sheet

It would be great if we could all log the info we need digitally. In real life, every pilot has a pen to hand to copy ATC info for flight safety. This is an attempt to address the stuff we need to make notes of, and still have space to take note of the really important stuff 'on the fly'. Please let me know how you think it can be improved?ff
